Tirzepatide vs. Semaglutide: Understanding the Difference

Both Tirzepatide and Semaglutide are highly effective GLP-1–based therapies.  While head-to-head studies show that Tirzepatide produces greater weight loss some patients do respond better to one drug over the other.  For patients who have plateaued on Semaglutide, or who are starting therapy for the first time and want the most effective option available, Tirzepatide is a compelling choice. That said, both medications are excellent options and the right choice depends on the individual.   

Candidacy and Evaluation

Tirzepatide is contraindicated in patients who are pregnant, have had pancreatitis, have a personal or family history of medullary thyroid carcinoma (MTC), or have been diagnosed with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

What to Expect

Tirzepatide is self-administered once weekly via a small subcutaneous injection, most commonly in the abdomen, thigh, or upper arm.  Medicine is dispensed a month at a time and we like to see patients back in office once a month so that we can monitor for side effects as well as customize your dose to help you best achieve your weight loss goal.  Most patients report a noticeable reduction in appetite within the first one to two weeks. Mild nausea is the most commonly reported side effect, particularly during the early dose escalation phase, and generally resolves within a few weeks. Serious side effects are rare.  

Long-Term Management and Maintenance

Obesity is a chronic condition that requires ongoing management.  Research confirms that patients who discontinue treatment are likely to regain a portion of lost weight, underscoring the importance of a structured maintenance strategy. For patients who have reached their weight loss goal, a custom maintenance plan is developed that involves ongoing medication management, routine lab work, and access to lifestyle and nutritional guidance to help ensure long-term success.
